Final Tax Checklist

 

Note:  Attach ALL income slips, and ONLY attach receipts if noted on info sheet.  Otherwise, complete amounts and keep receipts for your records only.

 

  • Change of Address – Please note any change of address, Phone numbers or email address

  • Dependants – We will use the information from last year.  Note any change in marital status or additional children

  • Have you Sold a Home in the past year? – Please advise us of this as we will need additional information regarding the sale of the home

  • All T4 slips

  • Investment income slips – Such as T5, T3, T4RSP, T4RIF, T4PS, T5008

  • Pensions – Old Age Security T4A(OAS), Canada Pension T4AP, other pensions and annuities T4A. 

  • Employment Insurance Benefits – T4E

  • Social Assistance / Workers Compensation Payments T5007, T5008

  • Childcare – with supporting receipts and SIN of the caregiver

  • Adoption costs

  • Tuition Receipts – T2202

  • RRSP Contributions Expenses on Information sheet:

  • Home Renovations (for 2009 tax year)

  • Child or Spousal Support – with copy of agreement if not previously provided

  • RRSP

  • Rent / Property Tax

  • Professional Fees or Union Dues (not included on T4)

  • Medical Expenses

  • Transit Passes (Monthly or longer passes only)

  • Charitable Donations (not included on T4)

  • Interest on Student Loans

  • Political Contributions

  • Tool Expenses

  • Moving Expenses – only if moving for work

  • Carrying charges and interest expenses (Safety Deposit Box or interest on investment loan)

  • Statement of Business Income and Expenses (Self employed individuals only)

  • Statement of Real Estate Income and Expenses

  • Statement of Employment Expenses and Declaration of Conditions of Employment (T2200)

  • Disability Tax Credit Certificate (T2201)

 

 

 

DON'T FORGET TO ATTACH YOUR VOID CHQ OR PRE-AUTHORIZED DEBIT FORM SHOULD YOU WISH FOR YOUR REFUND TO BE DIRECT DEPOSIT FROM CRA
